PT-141 (bremelanotide) is a melanocortin-4 receptor agonist that acts centrally in the hypothalamus rather than on the vascular system, making it mechanistically distinct from PDE5 inhibitors like sildenafil. It is FDA-approved for hypoactive sexual desire disorder (HSDD) in premenopausal women and used off-label in men with psychogenic ED or reduced libido that does not respond to vascular therapies. Onset is typically 30–60 minutes and effects can persist for several hours; it works independent of arousal cues, targeting the neurologic origin of desire itself.
