Tirzepatide is a first-in-class dual agonist that activates both the GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 receptors, engaging two complementary incretin pathways. The GIP arm enhances insulin sensitivity and lipid handling in adipose tissue while the GLP-1 arm suppresses appetite and slows gastric emptying. In the SURMOUNT and SURPASS trials it produced up to ~22% body-weight reduction and superior HbA1c control versus semaglutide, making it the current benchmark peptide therapy for obesity and type 2 diabetes with cardiometabolic comorbidities.
